Pelvic floor physical therapy involves the treatment of disorders involving the pelvic floor caused by tightness and/or weakness of the pelvic floor and surrounding structures. Treatment is focused on examination and evaluation of the cause of the dysfunction and the treatment thereof.
Dysfunction includes a variety of diagnoses including over active bladder (OAB) and incontinence. These disturbances can be caused by overactivity or poor relaxation of the pelvic floor. Often times, patient's find other issues accompanying incontinence such as constipation. Treatment of urinary dysfunction includes discovering the root cause of the issue and treating through exercise, education, and maintenance of the condition to improve overall quality of life.
Assessment of CPP includes targeting the multifactorial etiology. CPP can be a result of underlying conditions such as PCOS, endometriosis, fibroids, musculoskeletal abnormalities, painful bladder syndrome, IBS, and many other conditions that often require diagnosis from an MD. If you suffer from one of these conditions, pelvic floor physical therapy might be for you! Treatment targets the causes of pain including peripheral pain pathways and other pain triggers. Working on strengthening or down-regulation of tight pelvic floor are some of the many ways to address CPP.
Research indicates constipation as a risk factor for many other forms of pelvic dysfunction. Constipation treatment includes abdominal massage to the intestinal tract and colon to elicit motility in the GI tract, as well as strengthening of the pelvic floor and relaxation of the rectum. Treatment focuses on targeting causes for constipation to alleviate symptoms and avoid secondary progression of pelvic floor dysfunction.
Patients can benefit from pelvic floor PT both pre and postnatally. Treatement prenatally can assist in reduction of pain and preparing the mother's body for labor and delivery. Postnatal treament can assist in returning patient to prior level of function without pain. Some women experience Diastasis Rectus Abdominus (DRA) which is a separation in the abdomen due to weakness of the linea alba. Pelvic floor PT can address approximation of the separated structures and strengthening of the tissues involved. Perinatal treatment focuses on addressing muscle weakness and tightness of the spine and pelvis. Research also shows prenatal abdominal strengthening prenatally decreases the incidence of DRA postnatally.
SIJ pain can stem from weakness, instability, pelvic dysfunction and a multitude of other issues. Treatment focuses on addressing the root cause of pain and education on management of symptoms. Strengthening targets the hip, core and low back to aid in overall stability and pain management.
